Second mandibular molar right side (31) Asymptomatic during years, endodontic treatment performed several years ago Appearance of pus secretion without pain in the last 2 weeks No mobility Large periodontal pocket surrounding the tooth
Treatment plan
Atraumatic extraction and alveolar socket filling
Materials
Chorionic gel (Human Connective Tissue Matrix, HCTM: human placental extract) Bone substitute allograft: cortical mineralized-demineralized blend Amnion-chorion allograft membrane

Orthopantomogram Moderate radiolucency at 31
Probing reveals a 12 mm pocket depth on the buccal and distal sides.
Observation of the alveolar socket after atraumatic extraction confirms the bone loss.
Preparation of the putty consisting of Orafyl / Interfyl and Bone substitute allograft. Note the consistency and the easiness to mix and handle.
Insertion of the putty into the alveolar socket.
